Terms and Conditions

We are always pleased to assist our clients whenever we can. This document gives details of our terms and conditions of service. If, however, you have any queries or need clarification, please contact us and a member of staff will be happy to help you.

Please be aware that any information provided through any part of our social media platforms, including our website www.brentwoodsmiles.co.uk is for guidance & informational purposes only and does not constitute professional advice. No professional advice can be given without a clinical consultation with a dentist. For professional dental advice we strongly recommend that you see a dentist for consultation.

Devonshire House Dental Practice (24a High Street, Brentwood CM14 4AB) does not have a contract with a NHS Primary Care Trust and all treatments are provided on a Denplan and private basis only.

Treatment planning:

Once your treatment plan has been agreed with the Dentist, we will provide printed details of your plan. If this plan changes due to radiographic or clinical findings, we will inform you and discuss this with you. Treatment plans are valid for net 30 days from the date the treatment was prescribed. If there are any points on your treatment plan that you wish to query, please do not hesitate to ask us, before any appointments for actual treatments are made.

Consent forms:

Certain treatments require completion of a written consent form. This is in order to explain the treatment, aftercare and any risk to you thoroughly, before any of these treatments are carried out.

Payments:

Payments for treatment are due on the day the treatment is provided. Devonshire House Dental Practice does not operate a credit account system and we require fees to be settled at the end of your appointment. In some circumstances, we reserve the right to request payment in advance for certain treatments. Payment methods accepted are:

  • Cash
  • Debit/Credit Card including American express
  • BACS payment by arrangement

Where treatment incurs a laboratory fee, at least 50% of the total fee is due at the appointment where impressions are taken. Fees for certain treatments like Dental Implants and Invisalign® are taken in staged payments at each visit. We will discuss and plan a schedule of payment for you.

In cases where treatment is paid for on finance through a Finance company, please be aware that the finance agreement must be accepted and signed prior to the start of that treatment. Please note that finance is only available on treatments over £400, not including Teeth Whitening or Emergency Dental Treatment, and that a minimum deposit may also be required.

If you have financed any of the costs of your treatment through the finance company, and wish to cancel your treatment after signing the agreement forms, please be aware that a cancellation fee of up to 15% of the total finance amount will be charged.

The fees or offers displayed on any pages of our website are for guidance purposes and we reserve the rights to change them at any time. For updated prices you should call our reception team on 01277 217727. The cost of the treatment may vary depending of the complexity of the case and suitability of the patient. The patient will be provided with a full written treatment plan on their consultation with the dentist.

Devonshire House Dental Practice reserves the right to charge time based deposits for booking future appointments. Deposits are deductible off the cost of treatment.

Devonshire House Dental Practice reserves the right to make a charge for any debt passed to a debt collection agency.

All the fees applied for any form of Dental treatment at the clinic are based on; the materials used, laboratory costs incurred (if applicable), and the clinical time spent. The fees are however, demonstrated as treatments prescribed on the treatment plan for your information.

Late cancellation or missed appointments:

Devonshire House Dental Practice reserves the right to charge up to £100 per hour, in the event of a missed appointment or an appointment cancelled with less than 24 hours notice. For appointments longer than 1 hour, we require at least 48 hours advance notice.

Late for appointments:

We understand that some patients travel long distances to get to the clinic, and in some cases being late for appointments can be unavoidable. If you are late more than 30 minutes, please be aware that you may be asked to reschedule your appointment.

Cancellations and Failed Appointments:

We require a minimum of 24 hours notice for cancellation of any arranged appointment. When we schedule an appointment for a patient, we are booking the surgery time off for that patient’s treatment. Failure to give the appropriate notice of cancellation will result in a Failed Appointment Fee being charged. This fee is proportionate to the length of the appointment failed and is to cover the cost of the surgery time wasted.

Failed Appointment Fees must be settled before any other appointment is offered. Please note that we cannot accept notice of cancellation by e-mail or text message.

Guarantee:

At Devonshire House Dental Practice our Ceramic Veneers, Ceramic crowns and Implant crowns are guaranteed for a period of twelve months following placement, provided that the following conditions are met:

  1. The patient has fully paid for the treatment and does not owe the practice any money for the treatment received.
  2. The Veneers, Crowns & Implant crowns have not been damaged as a result of an accident, trauma or excessive grinding.
  3. The patient has followed all post treatment maintenance recommendations made by our dentists.
  4. No treatment is guaranteed for more than 1 year.

Some treatments may have a guarantee of less than 1 year, and in this case you will be informed by your Dentist either verbally or in writing, or both.

Personal Details:

It is very important that you give a full medical history and details of any medication you take. Should these change in any way, it is very important for you to tell your Dentist. It is the patient’s responsibility to inform the clinic of any changes in either personal details and/or their medical history.

Promotional use of Sales Vouchers, Gift Cards & Special Offers

Any company, partnership, individual, customer or other legal entity (“you” or “your”) purchasing goods and services in conjunction with vouchers/leaflets/forms/gift cards or any other promotional offer or deduction or discount together called the (“Promotional offers”) from Devonshire House Dental Practice (“us” or “we”, as applicable) does so subject to terms and conditions below:

Gift Card/Voucher:

The Promotional offers, issued by Devonshire House Dental Practice (the Promoter) can be exchanged for the promotional value indicated on the face of the card, providing all rules, terms & conditions are met:

  • This promotion is covered under United Kingdom Law.
  • The promotional offer is only valid in Devonshire House Dental Practice and cannot be redeemed online at www.brentwoodsmiles.co.uk
  • The voucher may only be redeemed against other single transaction products and services within Devonshire House Dental Practice only.
  • The voucher may not be used in conjunction with any other offer or promotion.
  • Change will not be given on any purchase and the voucher may not be exchanged for cash.
  • Offer is subject to availability.
  • The promoter can withdraw the offer at any time without prior notice.
  • Voucher is only valid between the promotional period/s (dates – from – to).
  • Only one voucher may be redeemed per customer/patient per transaction.
  • A transaction constitutes a single purchase of a single Devonshire House Dental Practice treatment, within a four hour period.
  • Vouchers lost or stolen are at the customer’s/patients’ own risk

Terms Governing the Cards/Vouchers

You may not sell the promotional offer Cards/Vouchers to any third parties without our written consent.

If we do give our consent then such consent may be subject to any conditions which we impose.

In any event you may not sell the promotional offer Cards/Vouchers through the internet without our express written consent, which you must obtain before making any such sale. We reserve the right at all times to refuse to permit any sale of the promotional offer Cards/Vouchers by you to a third party

Use of Images and X-rays:

Devonshire House Dental Practice may use images and x-rays of your smile and teeth only, for marketing and educational purposes on the Devonshire House Dental Practice website, and on promotional and educational literature. Your name will never be published, and identity will never be disclosed. However, if you DO NOT wish for us to use your images and x-rays in this way, please let us know.

Use of patient contact details:

At Devonshire House Dental Practice the health of our patients is our highest priority, and we also like to keep our patients informed of various important changes at the clinic and of our latest special offers. We like to remind our patients of their appointments, when they are due for appointments, and other various important reminders. On this note, you may be periodically contacted by the clinic via phone, text, email or by letter in the post. If you DO NOT wish to be contacted by the clinic by any or all of these means, please let us know.

Complaint’s policy:

At Devonshire House Dental Practice we always take complaints about any aspects of our services very seriously, in order to ensure that every patient has only the very best experience at all times.

Complaints can be made in writing by filling out a simple complaints form available from reception, or via email through our online website contact form, by the patient or by an authorized person on the patient’s behalf. Complaints should be made to the ‘Devonshire House Dental Practice Management’, and should be clear, so that they can be dealt with efficiently.

Every complaint will receive immediate written/email acknowledgement, and the Management will strive to resolve the complaint within a quick, reasonable period of time (usually about 2 weeks). For our full complaints procedure and policy, please ask a member of our reception team. If a complaint is about any aspect of clinical care or associated charges it will normally be acknowledged by the Management team and then referred to the treating Dentist. Please note that in these cases, Devonshire House Dental Practice accepts no liability on behalf of the treating Dentist and acts as a liaison between the patient and the treating Dentist, only.

If you are unsatisfied with our internal handling of your complaint, you are free to forward your complaint to the Dental Complaints Service (www.dentalcomplaints.org.uk). If still unsatisfied, you may wish to forward your complaint to the General Dental Council.

No tolerance/Abuse policy

At Devonshire House Dental Practice we operate a zero tolerance policy to abuse to our Dentists and staff, loud/disorderly/drunken behaviour, persistent missing and late cancellation of appointments (after multiple warnings). In these situations, Devonshire House Dental Practice reserves the right to refuse treatment and admission.

Use of Devonshire House Dental Practice Brand

You may not use any product image, corporate title, designation, service mark, trademark or any other trading name or style of Devonshire House Dental Practice without having first obtained our consent in writing.

You may not dispatch any flyers/brochures or any promotional materials to any third party if we have not previously approved any artwork incorporating the Devonshire House Dental Practice in writing.

We may withdraw any consent or approval referred to in this clause at any time by notifying you of such withdrawal.

Data Protection Act:

We store all patient personal details on a secure system in accordance with the Data Protection Act. All clinical notes, digital radiographs, digital photographs etc. remain the property of Devonshire House Dental Practice. Copies of notes, radiographs and photographs can be made available on request, and Devonshire House Dental Practice reserves the right to charge an administration fee for these.
